To apply for participation in the program, please work with your prescriber to complete the Patient Assistance Program Application. Be sure to complete the application in its entirety. Incomplete and/or unsigned applications may result in a delay of processing.

If your application is approved, the Testim Patient Assistance Program will be able to provide you with a one-year supply of Testim at no cost.
